How do I create a new form module from an existing module upon my website? |

- Login to the Insurance Website Builder Administration Console
- Click the Modify Forms Configuration link
- Click the Launch Form Builder button. The Form Builder is only available on the Gold or Platinum plan.
- Select the Line of Business this form applies to. Please contact the support department to add additional lines of business.
- Type the name of the form into the Form Name - English textbox.
- Tab to the Form Name - Spanish textbox and the site will automatically translate the form name to Spanish. Should the translation be incorrect you can correct it as needed.
- Select the Form Type. This will help classify the form on the website.
- Click the Fields tab.
- Select the form that you want to copy from the Copy Existing Form drop down box.
- Click the Copy Existing Form button.
- Confirm your selection by clicking Ok. This will clear your existing fields.
- Double click, drap and drop or arrow button the fields you want upon your form into the right listbox.
You can filter the displayed available fields by selecting Field Focus and Field Type drop down boes.

Allows you to view System Defined fields vs. User Defined fields
- Field Type
Allows you to view certain form field types.
- You can add custom form fields by clicking the Add Custom Form Field button.
- Once you have added all of your desired form fields click the Form Instructional Text tab. This tab allows you to define the instructions given to the user explaining the form's usage. Default text is provided for you.
- Click the Form Preview tab to view your completed form.
- Click the Save Form button when finsihed. You can also click the Cancel button to abandon your changes.
- Expand the line of business for which you just added the form.
- Click the box next to the custom form that you want to include on your site.
- Once correclty selected the box should be populated with a green check box Example :
