1. Login to the Insurance Website Builder Administration Console
2. Click the Form Submission History link
3. On the following screen you will find a list of current submissions categorized by each line of business.
4. Click the line of business that you are interested in to expand.
5. Click the form type that you want to view.
6. Click the  icon next to the submission you would like to view.
7. Your selection will be displayed.

A direct link to the form submission will be sent via email to the resposible party setup for that module.

All submissions are kept for thirty (30) days.  Afterwhich, all submissions are permanently deleted.
